Terms & Conditions — Staff Portal
Chugh Eye Surgery Centre, 637, Model Town, Ludhiana · Version v1.0 · Effective 24 August 2026
1. About this portal
Chugh ERP is the internal hospital management system of Chugh Eye Surgery Centre, 637, Model Town, Ludhiana. Access is restricted to authorised staff members only.
Staff accounts are created either by the administrator or through self-registration. Self-registered accounts remain inactive until an administrator verifies and approves them.
2. Account credentials
Your username is your registered mobile number (for self-registrations) or the staff code issued by the hospital. Passwords are 6-digit numeric PINs.
You are responsible for keeping your PIN confidential. Do not share it with anyone, and do not let others use a session opened with your account.
If you suspect unauthorised use of your account, report it to the administrator immediately. After 3 wrong PIN attempts the login is locked for 90 seconds as an anti-abuse measure.
3. Forgotten passwords
There is no public password reset on this portal. A logged-in user can change their own PIN from Account → Setting. If you cannot sign in, contact the administrator, who can verify your identity and reset your PIN from the admin panel.
4. Patient data & confidentiality
All patient information visible in the system — registrations, OPD/IPD records, discharge summaries, cashless documents, bills and reports — is strictly confidential.
Access patient data only when required for your official duty. Copying, sharing, screenshotting or exporting patient data for any non-official purpose is prohibited and may attract disciplinary and legal action under applicable laws including data-protection regulations.
5. Acceptable use
Use the system only for hospital work. Attempting to access other users' accounts, probing system security, altering records without authorisation, or introducing harmful code is strictly forbidden.
Internal chat and pin-board messages must remain professional. The hospital may review system activity for security and quality purposes.
6. Approval & termination of access
The administrator may approve, reject, deactivate or remove any staff account at their discretion — for example when an employee leaves the organisation. On separation, all access must stop immediately.
These terms apply from your first acceptance and continue to govern you for as long as you hold an account. The hospital may update these terms; continued use after an update constitutes acceptance.
Questions about these terms? Contact the administrator at the hospital front office.
